what should I do with all those bruises?

Bruises after cosmetic procedures are very annoying, aren’t they?

It is by far the most common complication associated with procedures such as dermal fillers or botox injections. There are many causes leading to bruising with varying severity and healing time. Studies have indicated that the incidence of bruising can occur between 24% to 68% in patients.

What makes us bruise more: Many prescribed medications, such as aspirin, ibuprofen, naproxen, warfarin, non-Vitamin K-dependent oral anticoagulants (e.g., dabigatran, apixaban, and rivaroxaban), heparin, affect blood clotting and will increase the risk of hemorrhage and bruising. If aspirin is being taken for analgesia, it should be avoided for one week prior to the treatment being performed. Similarly, non-steroidal anti-inflammatory (NSAID) medications, (e.g., ibuprofen, naproxen, diclofenac, celecoxib, and meloxicam) should be avoided for a similar period of time. Corticosteroids will also increase the risk of bruising as they increase the fragility of capillaries within the skin.

Herbal and vitamin supplements: Over the counter herbal and vitamin supplements have been known to increase the length of clotting time leading to a greater risk of bruising after procedures. High-dose Vitamin E, gingko biloba, and St. John’s wort, fish oils, omega-3 fatty acids, and garlic supplements can all lead to longer healing times and more bruising after aesthetic procedures. It is recommended to avoid such supplements for at least two weeks prior to surgical procedures.

 Management: To reduce the risk of unsightly bruising, it is recommended to cool your skin prior to injections. Cold compress and local compressions reduce bruising by inducing vasoconstriction. Another effective method is applying vitamin K8, bromelain or arnica to reduce bruising risk and increase the speed of resolution. Bromelain is an enzyme derived from pineapple that can aid the bodies disposal of metabolic wastes and speed the healing process following injury. It is normally taken at a dose of 200mg to 400mg, three times per day. Patients who develop bruising should be advised to stay out of the sun initially to limit the risk of persistent staining. Any extreme physical exertions or exercise can increase blood pressure and really aggravate potential bruises, so this should be avoided for at least 24 hours after your procedure. Patients should also be advised to avoid extended sun and heat exposure. Finally, if unacceptable bruising does develop, camouflage make-up may be applied.


Should I do Lip Augmentation?

It seems like plumping lips is very much in style right now. More and more people come in with a request to plump their lips, young girls want Kaylie Jenner lips: the juicier – the better, 

although “duck lips” or “fish lips” becoming less and less popular. Women (and some man) want NATURAL looking lips or just slight augmentation. A number one request I hear: “I want to plump my lips just please, don’t make them too big.”

It is true that we lose the volume of our lips as we age, so plumping the lips gives us youthful appearance.

Not overdoing it, keeping the right proportion of upper and lower lip and to other features of the face is key.

We got different tools to work with and different techniques.

The variety of fillers – Juvederm (Ultra, Ultra Plus, Volbella), Restylane (Silk, Refine) each material has its own qualities, which provides a unique advantage in lip augmentation. PDO threads (twist, smooth) can elevate the border of the lip. It is quite common to have lip asymmetry, which needs to be corrected.

Things to know about Sculptra™ Treatment

Sculptra has been introduced to the market of aesthetic medicine around a decade ago, but I have been cautious to start using it for a number of reasons:

1. It is more expensive (600-1200$$ per treatment) that our conventional fillers like Juvederm, Radiesse, Restylane.
2. Sculpture needs to be reconstituted 24-48 hours prior to injection, so canceling your appointment is not an option.
3.You do need to come at least 3 times 4-8 weeks apart, and results will start to show in a few months after starting the procedure. This is unlike all our other fillers which give instant results
4.After care, (vigorous massage of injection area 5 times a day for 5 days) it is very important for the prevention of complications, like subcutaneous nodules. Knowing myself, I would probably be calling my patients every day reminding them to massage their faces.
5. Sculptra can not be used in areas that require a lot of movement like around the mouth, nasolabial folds, marionette lines or around the eye, tear through area. However,
these are the areas that most people request treatment for. HA Fillers like Juvederm or Restylane are commonly used.
After a decade of practicing Aesthetic Medicine,  reading multiple reviews, and talking my Patients, I have decided to add it to my treatment arsenal, for very specific cases so careful patient selection is the key.

Is RADIESSE Safe and Effective

BioForm Medical, Inc. announced today that the U.S. Food and Drug Administration’s (FDA) General and Plastic Surgery Devices Advisory Panel has recommended the approval of Radiesse, a next-generation soft tissue augmentation product, for use as a filler material to restore soft tissue facial contours such as nasolabial folds and for use in the correction of lipoatrophy of the face associated with HIV therapy.

The Panel recommended approval of Radiesse with conditions in both indications following the review of data from a premarket approval application (PMA) for Radiesse to be used as an injectable device intended for use in the correction of facial lipoatrophy associated with HIV therapy; and a second PMA for Radiesse to be used as a filler material to correct facial lines and wrinkles such as nasolabial folds. Radiesse is the first filler to obtain concurrent positive recommendations for use in two facial aesthetic applications.

Non-Surgical Nose Job Brooklyn, Long Island

Non-surgical nose job (non-surgical rhinoplasty) is performed using injectable fillers restore a more symmetric look to your nose. Controlled and precise injections can make the nose look more proportional and stright. The results are instantaneous and there is essentially no recovery, unlike surgery. The most common filler used is Radiesse®, which is made up of calcium hydroxyl appetite crystals that are very similar to the mineral found in the bones of the nose, Juvederm or Restylane – Hyaluronic acid fillers also can be used.

Although it is a welcome alternative to surgery, thNose Correction before and after picturesere are some limitations to what this approach can accomplish.

There are many ways we can reshape the nose with fillers. The most common way injectable fillers can diminish the appearance of bumps on the nose. By injecting the bridge above the bump, it makes it appear straighter. This cannot, however, make a bridge smaller, or create a gently, feminine curve in the bridge that many women desire. While this creates a straighter nose it does not make it smaller. To create a smaller nose and remove the excess bridge a rhinoplasty (nose job) surgery is required.

In other patients who have a flat nasal bridge, fillers injected to create or augment the original bridge. This is a very common request in the Asian and African American population who often have flatter noses. Unlike the situation with nasal humps as described above, this treatment can give results similar to rhinoplasty surgery, because the surgery uses either your own cartilage or a permanent implant to build the bridge instead of a temporary injection. The difference between surgery and the non-surgical approach is simply that surgery gives a permanent result that does not require repeat treatments.

The nose can be further reshaped by filling dents, hollows or indentations.

In those who have had a prior rhinoplasty surgery and have irregularities, using a filler can be a welcome alternative to undergoing a more complex revision rhinoplasty surgery. This is especially true if the imperfections after surgery are minor (like small depressions in areas around the tip and bridge) and small refinements are only necessary.

Many patients want the droopiness of their nasal tip to be lifted with injections. A droopy tip of the nose can be cause by an overactive muscle that pulls down the nasal tip called the depressor septi muscle, especially when smiling. Botulinum toxin can be injected in between the nostril on the undersurface of the nose to release this muscle and create a more upturned nose. This will usually last 3 to 5 months at a time. Fillers can be further added where the nose meets the lip to support the tip at its base. Using both of these techniques together gives the best results, but the reality is that it lifts the tip by 2 to 3 mm in the best cases which is not very much. Because of this, the non-surgical approach is best reserved for patients who only require a small amount of lifting of the tip. Those with heavy or droopier tips will usually not see a significant difference to make it worth the expense, so rhinoplasty (nose job) surgery is a better option here.

While the entire procedure is relatively safe, those who are breastfeeding, pregnant or have neurological ailments must avoid Botox reatments If you’re not certain about your condition, consult with your physician first before taking any step further.

You should also be aware that temporary bruising is a common side effect of the treatment so there’s no need to worry about it. It’s also possible for patients to experience headache that may last 24 to 48 hours although this is rare. Eyelid drooping is another rare side effect that affects a small percentage of patients.
